If one Office app keeps showing an old or broken add-in, close Office, clear %LOCALAPPDATA%\Microsoft\Office\16.0\Wef, and add the add-in again. Use one server address per add-in across all deployment channels. After a new Outlook (olk) update the add-in may not start until Outlook is restarted.
